Check-in for the tournament will be on Wednesday, August 10th, 2011 from 5:00pm-8:00pm at the Renaissance Suites Hotel in Broomfield:

At check-in you will need to bring:
- 2 non-laminated copies of your CYS (or other state) certified roster.
- Medical release forms for all players (either the form below or the medical part of your CYS registration forms).
- Current / valid player cards for all players .
- Download the medical release form HERE.
- Download the guest player form HERE.
